I am working on incorporating a short survey that is displayed to the user on completion of a CustomBar (http://www.custombar.net) purchase. It'll just be the typical "Where did you hear of this product?" type questions, three or four of them and a comments field where the user can just write whatever.
I'm just curious what sort of results people that do this have received. What percentage of customers bother to fill them out? Have you received any negative response to the survey itself?

I'm interested to hear what other results people get since I'm planning to have a post purchase survey as well.
One thing I've found so far from doing beta/feedback surveys is that it's best to keep surveys as short as possible if you want to get a decent amount of responses. if your survey exceeds 5 questions, you'll probably want to offer an incentive.
For my post purchase survey, I'm planning to keep it short and sweet. Just 2-3 questions with drop-down menu options. I want the survey to be effortless as possible.

We recently did a "help us improve Digby's Donuts by filling out our fun survey!" in a recent newsletter. Users that fill out the entire survey by the end of August have a chance to win a free HipSoft Mega CD.
I thought we'd get about 20 responses but we got over 200 as well as a nice spike in Digby's Donuts sales. We were presently surprised and I'm sure giving away something for free is the main driving force. We also revved the version of the trial version which gives everyone a new 60 minute trial.

We use this same survey format for our external beta testing and it's proved incredibly helpful. The "what did you like most?" and "what did you like least" are always interesting.

I have mine up now, it's really simple and I'll probaby add to it as I come up with things to ask for. This was very easy to implement and it's a shame I didn't do it earlier. Already have helpful responses.

Resist the urge to add too much to it, or people will avoid the survey altogether. I remember hearing some rule of thumb on the number of questions, but I can't remember what that number was. 5-10 max? something like that.
